time tracking

I'm trying to figure out a way to use one of your products to do time tracking for employees. I'd like to have a simple form that an employee can use at the end of every day where he/she can enter the customer name, checkif they took lunch, etc and add unique fields like their odometer reading. At the end of the week, generate a report for each employee.

Thoughts on how to do this or best approach?
