I recently started managing a site for company that have this addon/component on their web. They now ask from me to make the form that will ask at the end for a user and pass and that way the form will be sent to an e-mail with subject with its username as prefix.
I understand PHP when I see it and get the way its working but I'm not coder so I need help with this. Is there a script that can be added so at the end of the form there is user and pass field that will need to be filled, to check if the pass is correct for that username and if it is to send the mail with shubjec like this: *username* sent you the application.

This can be done with a little custom scripting. You will basically need to add a hidden field that will store the username and place its placeholder in the email tabs (user / admin).
