Hi there and thanks in advance for taking the time to look into this post. My client has a Joomla! site that needs a multipage form built into it, so when I discovered RSForm! Pro and the tremendous functionality that it offers I was quite excited to get started. Part of the form my client needs is illustrated in the two images below:





Basically what my client needs is for users to be able to select individual teeth from the arrangement and have those selections indicated within the final form submission data. e.g. "Tooth Chart Implants : 3, 8, 9, 16, 19, 27, 31" for each of the teeth selected. Users should also be able to reset their selections via a "Reset" button if they wish to start over.

My question is this: can this be done within RSForm! Pro and if so, what might be the best way to go about doing so? I really do hope it is possible, however if it isn't I appreciate any and all suggested alternatives.
