As javascripting and code-writing is not my cup of tea I wondered if any of you 'guys' would be able to help me out with a form...
The purpose of the form is for employees to fill out a form every two weeks in an attempt to gather the necessary information for their wages:
First page consists of personal information, a selectlist for choosing the approbiate two-week period and two groups of radio-buttons.
The first group consist of three choices:
1. Have been on vacation for one or some of the days in the chosen period
2. Have been on vavation on all days in the chosen period
3. Have been working all days in the period.
The second group consist of these choices:
1. Have been sick on one or more days in the period
2. Have been sick all days in the period
3. Have been working all days
The main form on page two consists of these columns:
1.: Working at customer
2.: Number of working hours
3.: Overtime
4.: Sicknes
5.: Vacation
If radio button 2 - on the first page - is checked it means that the user have been on vacation for the entire period - this should result in a submission stating '75 hours vacation' for that period. And a thank you-note.
The same goes for choice no. 2 in the second group.
(It shouldn't be possible to check the two button 2's at the same time as you can't be both sick and on vacation in the same time.)
If radio button 1 - in either of the two groups are checked the user is presented with the approbiate columns on page 2. Eg. Button 1 in the first group and button 1 in the second group, button 1 and none in the second should result in page 2-form consisting of these columns:
1.: Working at customer
2.: Number of working hours
3.: Overtime
4.: Sicknes
or/and
5.: Vacation
If the user have had no vacation and had had no days sick, he will be presented with these columns only
1.: Working at customer
2.: Number of working hours
3.: Overtime
I hope it makes sense. Otherwise write me and try to explain my needs. And we could also discuss any compensation in a private matter.
