TOPIC: Checkbox Value and File Upload

Hi i created a form and but when i was submitted it only the the textbox values showed up in the manage submission planel. The checkboxes i created and the files i uploaded with the form did not show up. You're seeing the name of the checkboxes but there is no data below Any suggestions? thanks

As for the upload field, please edit it and test to see whether you can perform uploads or not.
