TOPIC: Linking image to another page

I created online registration which has been working for a long time now I needed to create a new form. I have an image that I link to the page of the form and been using this

index.php?option=com_rsform&formId=65

now when I click on the image which is linked to the above I get a blank page.

what am I doing wrong or missing..

Currently parents are enrolling their children online for programs and I need this fix NOW!
