I am trying to build a form that resembles an Employment Application. (Meaning my client want the emailed form results to look just like the paper form. This paper form has boxes where you write in the information.

How can I make a box and then populate the box with the correct info.
I do know how to make the field populate, but not how to mafe a box in html.

Does anyone have a finished form with boxes they could share with me, so I could see how to do it?
