TOPIC: Constant Contact Multiple List Selection

I would like to create a Constant Contact signup form that allows for a user to select from a checkbox which of the mailing lists the would like to signup for.

We have our main list which it saves to but would like to allow a user to select several other specific lists such as the following:

Preschool
Elementary
Middle School
High School
Adult

etc etc.

When I create a new form with the checkboxes it does not pass the values over to Constant Contact.

Any guidance on this would be appreciated.
The administrator has disabled public write access.
