I will try to describe my problem, and I hope that you will understand me.

I have to put about 20 different articles on my site, and each has to have a simple form at the end that allows visitors to give us their suggestions, comments, criticism...

This form is the same for all articles, BUT when visitor submit - we need to know from which article he submited form, on which article the form refers.

Since the forms on these articles look exactly the same, is it possible that I create just one form and use it on all those articles, but when user submit it - I know from which article he did that?
