My form layout needs need 4 Columns 2 lines.

Can this be done?

Do I need to download a pluggin?

How can I do this?

Here is an example for one of our quote request items. when some wants a quote they need to give us the number of Tons, steel Size, steel Grade, special instructions Notes.

They do this for several "different" items.

Does this make sense. This is the reason for the 4 columns.
