TOPIC: Checkbox item WITH textfield?

I'm trying to create an order form that someone can fill out online. They need to "check" the item and then put in a qty.

I know I can just make a whole bunch of textfield fields and use them for the qty but I wanted to look at other options.

Thoughts?

The form I'm trying to replicate is:

I'm not looking to do calculations or take an actual order online right now. They need manual checks so just the form submit with as much actual data as I can get.
