Hi

I know this should probably be simple but I am having trouble....

i want a form with 20 questions in a table and then a yes & no checkbox and text field at the bottom they can put their total. No validation, no submission more visual so they can complete online. I can create the fields but i dont know how to put the content in. I have looked through the forums but I must not be asking the right questions.

I've attached a sample file of what I want, if you could please tell me how to get the content in would be greatly appreciated.

I'm sure it could be done but this would take some time to work out.

Just use a single column for your form fields and put this in each field's style box.
